Our highly trained Registered Nurses will perform a comprehensive assessment and evaluation within 24-48 hours upon receiving the referral from your Physician. Within this evaluation, our RN’s will determine the need of services such as Physical Therapy, Occupational Therapy, Speech Therapy, Medical Social Worker and Home Health Aide services. Our RN’s provide quality nursing care in the convenience of your own home, in accordance with the patient’s plan of care and state and federal regulations. Our RN’s will assess physical condition; health promotion and disease prevention; coordination of services; teaching and training activities; and provision of direct nursing care. Licensed Vocational Nurse works closely with the assigned RN, Clinical Manager and Director of Nursing. With enhanced communications and level of acuity, the team coordinates on a daily basis to trend events that leads to prevention of any exacerbation and hospitalization episode.
